Merced United Medi-Cal Play Program

Program Overview: Merced United is committed to promoting sport equity and ensuring that all community members have the opportunity to play and grow through soccer. Partnering with the Central California Alliance for Health, this program aims to reduce financial barriers for families on Medi-Cal and engage adult members in physical activity. The initiative supports participants in both our Merced United Recreational Soccer program for youth and our Merced United League for adults, reimbursing registration fees upon the completion of a sports physical. Enroll yourself or your child in our Merced United soccer programs, and once you/they complete a physical with one of our partnered health centers or other health care provider, we will reimburse the registration fees. It is important to note that your immigration status does not matter and is not affected when applying for Medi-Cal services! How to Participate:

Register: Enroll yourself or your child in Merced United’s Recreational Soccer, Competitive, or League programs. Full fee must be paid to qualify.
Complete a Physical: Schedule and attend a sports physical examination with one of our partnered health partners, or with any Health Care Provider of your choosing that serves Medi-Cal members.
Submit Proof: Email the following to christopher@mercedunited.com
1. Proof of Medi-Cal membership (Medi-Cal card with your name clearly on it, or equivalent), and
2. Proof of the completed (corner screenshot of After Visit Summary or equivalent. Do not share personal information).
Complete Program Feedback Survey: Program feedback allows us to gather insights from our community and improve our programs
Receive Refund: A $75.00 Refund Checks will be provided to you on the field after the 5th game of the season and before the championship game. This is all dependent on you following steps 1-4 above.
